Implant Options
Patients can choose from three basic types of implants: saline, silicone or cohesive gel implants, also known as gummy bear implants. Each type of implant has its benefits and drawbacks:
- Silicone Implants: less costly and require a smaller incision, but they also tend to be associated with a higher rate of breast rippling and if the implant ruptures, the evidence is visible almost immediately
- Saline Implants: tend to look and feel more like natural breast tissue, but require a bigger incision to place and require regular MRIs to ensure no rupture or leakage has occurred
- Cohesive Gel: innovative silicone implants that hold their shape better than all other implants and tend to have a more natural look and feel
Each of these implants come in a range of shapes and sizes to accommodate every profile and desired result. You and your plastic surgeon should work together to determine which implant is the best choice for you.
Incision Location
The placement of your incision will be based on the type of implant you choose and your personal aesthetic goals. Choices include:
- Around the areola, the dark pigment that surrounds the nipple
- In the breast crease, where scarring will be hidden in the natural folds of the skin
- In the armpit, used for saline implants that are filled after they are placed in the breast tissue
No matter where your incisions are located, Dr. Sherman will ensure that any visible scarring after the procedure is minimal. In most cases, thanks to skin pigment or natural skin creases, your scars will not be easily noticed and will definitely be hidden under a bikini or underwear.
Implant Placement
You will also get to choose where your implants are placed. For some patients, that placement is directly in the breast tissue. For others, the implants are placed underneath the pectoral (chest) muscle. The decision on placement will be determined by a number of factors, including the chest width, your age and your profession. For example, body builders will frequently choose to have implants placed above the muscle to avoid an irregular appearance during weight lifting.
Anesthesia
Breast augmentation is generally performed on an outpatient basis with the choice of either general anesthesia or local anesthesia with sedation. The right type of anesthesia for you will depend on your medical history, the specific procedure you are going to have and the placement of the breast implants. Usually patients that have the implants placed under the pectoral muscle do better with general anesthesia.
